Title: In Loving Memory

I've been looking everywhere for any kind of rememberance plaque, in loving memory, or such.    One that the name can also be added to.    Thanks for your help.

I'm not really sure what you are looking for but if you provide a photo and dates of birth and death, I could probably come up with something.
