Title: Show Choir Pattern

Can anyone help with this.I need it for my daughters school.
I would like to have
"Show" on top
with some music notes in the middle
"Choir" on the bottom.
Any help with this would be greatly appreciated.

It is perfect.Now I have to find time to cut some, or make 1
and take orders.
